The nephew of Mexican drug lord Rafael Caro Quintero was extradited from Mexico to the United States Wednesday on charges of participating in a large-scale narcotics distribution conspiracy and using one or more firearms in connection with narcotic offenses.
51-year-old Ismael “Fierro” Quintero Arellanes was arraigned on Thursday at the federal courthouse in Brooklyn, New York. He was ordered detained pending trial.
Head of Security
The narco underworld is one inhabited by snakes and rats, each one willing to bite your throat to get one step higher on the ladder. No wonder then that many kingpins rely on family for protection. If you can’t trust your own blood, who can you trust?

No surprise then that Quintero Arellanes served as his uncle’s lieutenant and head of security. In that role, he served as personal bodyguard, relayed messages and orders between Caro Quintero (right) and other members of the cartel to facilitate drug trafficking activities, and protected Caro Quintero to enable him to avoid capture.
Heroin, meth, coke & marijuana
Being part of his uncle’s organization, Quintero Arellanes is charged with participating in an international heroin, methamphetamine and cocaine distribution conspiracy from February 2015 through June 2018 and an international marijuana distribution conspiracy from January 1980 through June 2018, as well as using firearms in relation to his drug trafficking organization.
“As alleged, the defendant was a top lieutenant and head of security for Rafael Caro Quintero, an infamous drug lord whose violent organization has for decades sent thousands of kilograms of drugs to the United States,” stated United States Attorney Breon Peace. “Dismantling the leadership of drug trafficking organizations that have wreaked havoc here and abroad is a top priority for our Office.”
Rafael Caro Quintero was arrested in Mexico on July 15, 2022, and is pending extradition to the United States. He is currently imprisoned in the Altiplano maximum security prison in Almoloya de Juárez.
The day of his arrest, the U.S. asked for his extradition. It wants to prosecute him for the torture and murder of DEA agent Enrique Kiki Camarena in 1985.
